Module 3 - Screening/Testing
This final module, Screening for Perinatal Depression, brings together information from Modules I and II as it relates to the screening process. Perinatal depression is defined in more detail and the differences between minor and major depression explained. The prevalence of depression among women with Medicaid-paid births is examined. Screening challenges are explored, including possible cultural barriers, and providers learn to use sample questions to help them evaluate the presence of risk factors. The use of the Edinburgh Postnatal Depression Scale, a free, no-cost, self-administered detection tool, is explained, including a demonstration of how it is scored. If the presence of depression is confirmed with the Edinburgh, providers learn how to follow-up with the patient Health Questionnaire for Depression, or PHQ-9, to determine its severity. Scoring of the PHQ-9 is demonstrated, and referral to possible services discussed. Providers are given critical resources for dealing with positive scores for possible suicide in both measures.
This module includes clips of interviews with African American and Hispanic women talking about their depression and then being screened. A downloadable video of Dr.
Linda Oakley, a nurse researcher and clinician describing the African American experience with depression is also included, as is a downloadable review of the literature on depression and women of color, authored by Dr. Kang, and a supplemental list of suggested articles for further reading.
The producers of this training program acknowledge that the age of the child referenced in the interviews exceeds the age limit for First Steps.
